School Registration Information
Please fill out the Registration Forms below and submit the following items to the school office as soon as possible to or your place may be given to another child.
- Kinder Pre-School Report
- Last or most recent Report Card
- Birth Certificate
- Baptismal Certificate
- First Communion Certificate (if applicable)
- Immunization Records
- Kindergarten and Grade 1 Students need Doctor's Health Examination Report to be filled out. A copy of the form may be obtained from the school office.
